爪の小さな白い点の多くは、軽いケガやマニキュアなどで爪が白く見える状態。カルシウム不足や幸運のしるしという説には根拠がない。
ひとことで言うと爪の白い点は、だいたい小さなケガのあと。栄養不足とは限らない。
爪に、ぽつんと小さな白い点。子どものころ、「カルシウムが足りない証拠」「幸運のしるし」などと言われた人もいるでしょう。
でも皮膚科の教科書を開くと、どちらも出てきません。爪の白い点には、ちゃんとした名前と分類があります。
01名前は「白斑(はくはん)」=leukonychia
爪が白く見える状態を、医学用語で白爪(leukonychia、ロイコニキア)と言います。
よくある小さな白い点は、真の白爪の一種で、専門的には点状白斑(leukonychia punctata)と呼ばれます。
02小さな白い点の正体
爪は、爪の根もと(爪母)で作られます。このとき、
- ぶつけた、はさんだ、強く押したなどの軽い外傷
- 爪をいじるくせ、きつい靴
- マニキュアや除光液など、爪の表面を傷めるもの(この場合は偽性白爪)
などで、爪の層にすき間や未成熟な細胞が残ることがあります。すると、そこで光が散乱して白く見えるのです。
ポイントは、白い点が爪と一緒に伸びていくこと。これは「爪が作られたときの出来事」の記録だからです。

03一方で「爪が全部白い」は要注意
同じ「白」でも、爪全体や広い範囲が白っぽい場合は、話が変わります。
レビューでは、爪全体・一部・横線状に白くなる例として、
- テリー爪:爪のほとんどが白く、先に細い赤い帯が残る。肝臓の病気などで報告される
- リンゼー爪(ハーフアンドハーフネイル):根もと半分が白く、先半分が赤褐色。腎臓の病気などで報告される
- ミーズ線・ミュルケ線:爪を横切る白い線。重い病気や特定の中毒などで報告される
といったパターンが挙げられています。このうちテリー爪・リンゼー爪・ミュルケ線は、爪そのものではなく、下の皮膚や血流の変化を映しています。ミーズ線は爪が作られる過程の乱れの記録です。
見分け方の目安として、爪を押して白さが消える(血流の影響)なら見かけの白爪、押しても変わらないなら真の白爪などの可能性がある、という方法が使われます(例外もあります)。
04どんでん返し:まれに「遺伝する白い爪」もある
ほとんどの白い点は心配のいらないものですが、例外もあります。
生まれつき爪全体が白い遺伝性の白爪が報告されており、家系内で受け継がれる例が知られています。また、爪の下の小さな腫瘍(良性)が白い線として現れることもあります。

Most small spots form when the root of the nail is slightly injured, for example by a bump or by biting the nails. This leaves tiny gaps in the nail layers, which scatter light and look white.

White flecks on fingernails are often blamed on a lack of calcium. Dermatological reviews give that idea no support. In 2022, Iorizzo and colleagues sorted nail whiteness, known as leukonychia, by where the white sits. In true leukonychia the nail plate itself is white.
In apparent leukonychia the plate is normal, but changes in the tissue underneath make it look white. In pseudo-leukonychia only the surface is affected, as can happen with nail polish. The familiar little dots usually belong to the first group.
A knock to the nail root leaves immature cells or tiny gaps, and these scatter light. Fingernails grow roughly 3.5 millimeters a month, so a dot drifts toward the tip over several months, a record of an injury you have probably forgotten. Whiteness across most of the nail is another story.
Patterns called Terry's nails and half-and-half nails have been reported in liver and kidney disease, and they come from changes in the nail bed, not the plate. Rare inherited forms also exist, so widespread whiteness deserves a doctor's examination. Pressing the nail gives a quick clue. If the white fades under pressure, the change lies in the tissue beneath, which marks the apparent type.
