誕生日が1週間違うだけで認知症リスクに差? 帯状疱疹ワクチンの「自然実験」

ひとことで言うと誕生日の違いで生まれた「ワクチンの差」から、認知症との関係が見えてきた!
「ワクチンを打つと認知症になりにくい」。もし本当なら、とてつもないニュースです。
でも、こういう話には落とし穴があります。ワクチンを打つ人は、もともと健康に気をつかう人が多いかもしれない。だとしたら、認知症が少ないのはワクチンのおかげではなく、生活習慣のおかげかもしれません。
2025年、アメリカの研究チームは、ある「偶然の境目」を使って、この問題をうまく避けました。
011933年9月2日という「境界線」
ウェールズでは、1933年9月2日より前に生まれた人は対象外で、一生その対象に入れませんでした。一方、その日以降に生まれた人は、少なくとも1年間、接種を受けられたのです。
その結果、ワクチンを受けた人の割合は、
- 境目より1週間だけ年上の人:0.01%
- 境目より1週間だけ年下の人:47.2%
と、まるで違っていました。でも、たった1週間違いで生まれた人たちが、健康意識や生活習慣でそこまで違うとは考えにくい。これが「自然実験」と呼ばれる理由です。
02認知症の診断が約2割少なかった
この境目の前後を比べる方法(回帰不連続デザイン)で分析した結果、ワクチンを受けると、7年間に新たに認知症と診断される確率が3.5ポイント下がり、割合にすると約20%少なかったと推定されました。
この効果は男性より女性で強く見られました。また、イングランドとウェールズの死亡診断書という別のデータでも、似た傾向が確認されています。

03なぜ帯状疱疹ワクチンが?
帯状疱疹は、子どものころにかかった水ぼうそうのウイルスが、体の神経の中に何十年も潜んでいて、年をとって免疫が弱ったときに再び暴れ出す病気です。
神経に潜むタイプのウイルスが認知症に関わっているのでは、という説は以前からありました。研究チームは、ワクチンがウイルスの再活性化をおさえることや、免疫全体への良い影響が関係している可能性を考えています。ただし、仕組みはまだ分かっていません。
さらに2025年12月には、同じスタンフォード大学のチームが、すでに認知症と診断された人でも、帯状疱疹ワクチンを受けた人は病気の進行がゆるやかだった可能性を示す研究を発表し、話題になりました。
04まとめ
「たまたま決まった誕生日の境目」が、ワクチンと認知症の関係を調べるための、またとない実験装置になりました。
すでに世界中で使われているワクチンに、思わぬ「おまけの効果」があるかもしれない。もし確かめられれば、認知症予防の新しい道になるかもしれません。今後の研究に注目です。

When a public shingles vaccine program began there in 2013, eligibility depended on date of birth. Anyone born before September 2, 1933, was never eligible. Anyone born on or after that date was. Two groups separated by a single week should be almost identical in lifestyle and health awareness, so any difference in dementia can reasonably be put down to the vaccine. The study has gaps. It looked at an older live vaccine, nobody knows how the shot might protect the brain, and a diagnosis depends on whether someone saw a doctor. Clinical trials would settle these questions. One more pattern showed up, though. The protective effect was stronger in women.
